Description of Jewelry Service

I am requesting a permanent jewelry service, which involves the application of a chain directly onto my wrist, ankle, or neck. The chain will be custom-fitted and permanently closed using a welding process. I understand that this is a permanent application, and the jewelry is designed to be worn continuously until it naturally breaks or is intentionally removed.

Risks and Aftercare

Risks:

• Skin irritation or allergic reaction to the metal.

• Discoloration of the jewelry over time.

• Potential for the jewelry to snag or break.

• Possible burns from welding if not kept still during application.

• Requirement to remove jewelry for certain medical procedures, such as MRI

Aftercare Instructions:

• Avoid harsh chemicals, excessive pulling, and abrasive cleaners.

• Clean gently with mild soap and water.

• Be mindful of snagging the jewelry on clothing or other objects.

• If any irritation occurs, remove the jewelry and consult a medical professional.

• Consider the need to remove it for medical procedures. It can be easily cut off with scissors or pliers. We can re-weld the bracelet for you for a small fee.

Consent and Release of Liability

I understand the nature of the permanent jewelry service, including the application process, potential risks, and aftercare instructions. I confirm that I am not allergic to the metals used in the jewelry (typically sterling silver, gold filled, or solid gold – please confirm with technician). I agree to follow the aftercare instructions provided to ensure the longevity and safety of the jewelry.

I hereby release [Linked By Love] and its employees from any and all liability for any injury, damage, or loss that may occur as a result of receiving the permanent jewelry service, except in cases of gross negligence or willful misconduct.

I grant Linked By Love permission to use photos and videos of my jewelry application for marketing purposes, including but not limited to social media, website, and promotional materials. I understand that my likeness may be visible in these materials.

This document outlines the permanent jewelry service, including client details, a description of the service, associated risks, and aftercare instructions. It also includes a consent and release of liability section, a release for photo usage, and signature/date fields for both the client and technician. The client acknowledges their understanding and agreement by signing this waiver.
